摘    要:
目的介绍非体外循环下冠状动脉搭桥术(OPCAB)的应用体会。方法自1999年1月至2006年11月,OPCAB 266例,年龄39—88岁,单支血管病变2例、两支血管病变20例,三支血管病变244例,LVEF26—67%。结果术后呼吸机辅助8.2±3.3小时,138例(51%)不需输血,一过性心房纤颤15例,伤口感染4例,低心排出量综合征3例,二次开胸止血2例,肺部感染1例,住院死亡4例(1.5%)。生存262例,症状消失或明显减轻,生活质量明显提高。结论OPCAB创伤小、恢复快、安全可行。

Abstract:
Objective To summarize our experience in off-pump coronary artery bypass grafting (OPCAB). Methods Between January 1999 and November 2006, 266 patients with coronary artery diseases, aged 39-88 years old, underwent coronary artery bypass grafting with off-pump technique. Of the 266 patients, there was single vessel disease in 2 case, double vessel disease in 20 cases, triple vessel disease in 244 cases, LVEF was 26% to 67%. Results The mechanical ventilation time was 8.2?.3 hours, 138 cases (51%) had no blood transfusion after operation. Two cases received reoperation for massive haemorrhage. Temporary atrial fibrillation occurred in 15 cases, wound infection in 4 , low cardiac output syndrome in 3, lung infection in 1. There were 4 hospital deaths (1.5%). All the 262 survivors had no symptom or significantly reduced symptoms with improvement in quality of life. Conclusions off-pump coronary artery bypass grafting appears to be minimally invasive, safe and effective with less complications.
