克罗恩病14例临床分析

目的 探讨克罗恩病的诊断与治疗方法、方法 回顾性分析1995年1月至2006年5月经外科治疗的14例克罗恩病资料。结果 全组患者以腹痛、腹泻和腹部肿块为主，腹痛腹块大多位于右下腹，其他还有瘘管形成、肠出血等表现。全组术前确诊仅7例（50％），均经手术治疗，其中回肠部分、结肠部分切除术各5例．回盲部切除术3例，小肠升结肠短路手术1例。术后完全缓解及有效11例．占78．6％。结论克罗恩病临床表现多样，术前较难确诊，严格掌握手术适应证和合理的外科治疗可提高克罗恩病的治疗效果。

Surgical treatment of Crohn disease (report of 14 cases)

Objective To evaluate the diagnosis and the treatment of Crohn disease(CD).Methods In this rctrospective study,14 patients with CD from January 1995 to March 2006 were collected.All of their clinical symp- toms,clinical characters,and surgical, treatment and other information were also analyzed.Results The CD cases mainly manifested abdominal pain,diarrhea and abdominal mass in the low and right of the abdomen,other clinical manifestation included fistul,intestinal hemorrhage.The confirmed diagnosis was 7 cases(50%).All patient's re- ceived surgical operations as following:partial intestinal enterectomy in 5 cases,colestomy in 5 cases.ileocecal resec- tion in 3 cases,small intestine and ascending colon kiss in 1 case.Completed remission rate and effective rate of medi- cal and surgical therapy were 11 cases(78.6%).Conclusion It is varied in clinical manifestation of Crohn disease and it is no easy to diagnose,the patient to receive surgical operationa must be control,rational medication and selec- tive surgical intervention can improve the outcomes of CD.
