腹腔镜下卵巢良性肿瘤剔除术中两种止血方式对卵巢功能的影响

目的比较腹腔镜卵巢良性肿瘤剔除术中两种不同止血方式对患者术后卵巢功能的影响。方法选取2010年7月～2012年7月在我院接受腹腔镜卵巢囊肿剔除术的卵巢良性肿瘤120例的临床资料．根据止血方式分成双极电凝止血组（66例）和缝合组（54例）。分别观察两组的手术时间、出血量、术后平均住院天数，术前、术后月经情况，卵巢功能早衰以及阴道B超探测窦状卵泡数（F0]。结果手术时间，术中出血量及月经情况、比较，缝合组显著优于双极电凝组，两组术后月经情况有显著性差异（P〈0．05或P〈0．01）；而在卵巢功能出现早衰比较两组无显著性差异（P〉0．05）。另外，两组术前窦卵泡数（F0）差异无统计学意义（P〉0．05），术后1个月和3个月时，电凝组F0显著降低（P〈0．01）。结论腹腔镜下卵巢囊肿剥除术后的缝合止血法比双极电凝止血法对卵巢功能的影响小，有助于提高患者的治疗效果，改善患者的生活质量，值得临床推广应用。

The influence of two kind of benign ovarian tumoreceomy in hemostasis on ovarian function after laparoscopic

Objective To explore the influence of two different ways of hemostasis during laparoscopic surgery for benign ovarian tumor on its function. Methods Analysis the clinical data of 120 cases who received ovarian benign tumor removed surgery during July 2010 to July 2012 in our hospital. The patients were divided into two groups electrocoagulation group （66 patients） and suture group （54 patients）. The operation time, blood loss, postoperation hospitalization duration as well as menstrual cycle, premature ovarian failure and number of antral follicles by vaginal B ultrasound detection. Results Compared with the suture group, the electrocoagulation group had shorter operation time and less blood loss （P 〈 0.05 or P 〈 0.01 ）. Significant differences were detected in the menstrual cycle between the two groups （P 〈 0.05）. After the operation, premature ovarian failure was detected in three cases from the electrocoagulation group , and one case from the suture group. Preoperative F0 had no significant difference between the two groups （P 〉 0.05）, F0 of the electrocoagulation group had significantly reduced after 1 month and 3 months later （P 〈 0.01 ）. Conclusion Suture method shows less influence on the ovarian function than electric coagulation.
