低功率60瓦钬激光前列腺剜除术治疗良性前列腺增生症的临床观察

目的:评价低功率60瓦钬激光前列腺剜除术用于良性前列腺增生治疗中的疗效与可行性。方法:回顾性分析2019年4月至2019年7月收治的30例良性前列腺增生患者临床资料,30例患者均采用低功率60瓦钬激光和组织粉碎器行经尿道钬激光前列腺剜除术(HoLEP术)和组织粉碎术。分别记录术前国际前列腺症状评分(IPSS)、术前前列腺生活质量评分(QOL)、术前最大尿流率(Qmax)、手术时间、切除前列腺体积;术后留置尿管时间、患者住院时间,术后国际前列腺症状评分(IPSS)、术后前列腺生活质量评分(QOL)、术后最大尿流率(Qmax)。结果:手术均顺利完成,术后IPSS、QOL、Qmax较术前明显改善(P<0.05)。术中、术后并发症方面无包膜穿孔,有1例输血,术后无尿失禁,有1例膀胱颈狭窄,予扩张后好转。结论:低功率60瓦钬激光前列腺剜除术可以用于治疗良性前列腺增生,且在基层医院更容易推广和应用。

Clinical efficacy of low-power 60-watt holmium laser enucleation of prostate in the treatment of benign prostatic hyperplasia

Objective:To evaluate the efficacy and feasibility of low-power 60-watt holmium laser enucleation of the prostate in the treatment of benign prostatic hyperplasia(BPH).Methods:The clinical data of 30 patients with benign prostatic hyperplasia admitted from April 2019 to July 2019 were retrospectively analyzed.All 30 patients underwent transurethral holmium laser enucleation of the prostate(HoLEP)and tissue morcellation using a low-power 60-watt holmium laser and tissue morcellator.Preoperative International Prostate Symptom Score(IPSS),preoperative prostate quality of life score(QOL),preoperative maximum urinary flow rate(Qmax),operation time,and resected prostate volume;postoperative indwelling catheter time,patient hospital stay,postoperative International Prostate Symptom Score(IPSS),postoperative prostate quality of life score(QOL),and postoperative maximum urinary flow rate(Qmax)were recorded,respectively.Results:IPSS,QOL and Qmax were significantly improved after operation(P<0.05).There was no capsular perforation in terms of intraoperative and postoperative complications,1 case of blood transfusion,no postoperative urinary incontinence,and 1 case of bladder neck stenosis,which were improved after dilatation.Conclusion:Low-power 60-watt holmium laser enucleation of the prostate can be used to treat benign prostatic hyperplasia,and it is easier to popularize and apply in primary hospitals.
