沐舒坦对早产儿呼吸窘迫综合征的预防作用

目的 :探讨沐舒坦对早产儿呼吸窘迫综合征 ( NRDS)的预防作用及其影响因素。方法 :采用沐舒坦针剂 ,每天 7.5 mg/kg,6h一次静脉滴注治疗早产儿 1 8例 ;每天1 5 mg/kg,6h一次静脉滴注治疗早产儿 46例 ,并与同期未用沐舒坦治疗的 64例早产儿进行前瞻性临床对照研究。观察是否发生 NRDS及影响预防效果的临床因素。结果 :预防组与对照组相比 NRDS的发生率分别为 34.4%、5 9.4% ,二者有显著性差异 ( P<0 .0 5 )。胎龄≤30周、体重≤ 1 kg、第 1次用药时间 >6h、剂量为每天 7.5 mg/kg、用药时间≤ 3d的早产儿较胎龄 >30周、体重 >1 kg、第 1次用药时间≤ 6h、剂量为每天 1 5 mg/kg、用药时间 >3d的早产儿预防效果差。结论 :沐舒坦对早产儿 NRDS的发生有预防作用 ,对胎龄 >30周、体重 >1 kg的早产儿预防效果好 ,最好第 1次用药时间≤生后 6h、剂量≥每天 1 5 mg/kg、疗程 >3d。

Preventive effect of mucosolvan on neonatal respiratory distress syndrome

Objective:To explore the preventive effect of mucosolvan on respiratory distress syndrome(NRDS). Methods:18 cases received mucosolvan treatment at dosage of [7.5mg/(kg.d),ivgtt,q6h]and 46 cases at dosage of [15mg/(kg.d),ivgtt,q6h]were enrolled as preventive group, another 64 cases who received no mucosolvan therapy were enrolled as control group. Results :The incidence of NRDS significantly reduced in preventive group(34.4%),control group was 59.4%.The factors,such as gestational age(≤30 weeks),body weight(≤1kg), first dosage(>6h)dosage[7.5mg/(kg.d)]?course(≤3d) influenced the efficacy of prevention of mucosolvan. Conclusion:Mucosolvan can reduce the incidence of NRDS in and had good preventive effeot for neonate. Specially first dosage premature infants with gestational age(>30 weeks)?body weight(>1kg),occasio(≤6h ) ,dosage[≥15mg/(kg.d)] ,course(>3d) .
