医院护士主导的慢性病团队管理模式对冠心病患者疾病管理能力及生活质量的影响

目的 探讨医院护士主导的慢性病团队管理模式对冠心病患者疾病管理能力及生活质量的影响。方法 选取我院2019年8月至2021年3月期间收治的90例冠心病患者,随机分为对照组和研究组各45例。对照组采用常规随访护理服务,研究组在对照组基础上采用医院护士主导的慢性病团队管理模式。对比两组患者干预后的疾病管理能力、生活质量评分及主要心血管不良事件发生情况。结果 干预后,研究组的疾病管理能力及生活质量评分均高于对照组(P<0.05)。研究组主要心血管不良事件发生率为8.89%,低于对照组的26.67%(P<0.05)。结论 医院护士主导的慢性病团队管理模式可明显提升冠心病患者的疾病管理能力,改善其生活质量,降低心血管不良事件发生率,值得临床应用。

Influence of Chronic Disease Team Management Model Led by Hospital Nurses on the Disease Management Ability and Quality of Life of Patients with Coronary Heart Disease

Objective To explore the influence of chronic disease team management model led by hospital nurses on the disease management ability and quality of life of patients with coronary heart disease.Methods 90 patients with coronary heart disease admitted to our hospital from August 2019 to March 2021 were selected and randomly divided into control group and study group,with 45 cases in each group.The control group received routine follow-up nursing service,and the study group received chronic disease team management model led by hospital nurses on the basis of the control group.The disease management ability and quality of life scores after intervention,and the major cardiovascular adverse events were compared between the two groups.Results After intervention,the scores of disease management ability and quality of life in the study group were higher than those in the control group(P<0.05).The incidence of major cardiovascular adverse events in the study group was 8.89%,lower than 26.67%in the control group(P<0.05).Conclusions Chronic disease team management model led by hospital nurses can significantly improve the disease management ability of patients with coronary heart disease,improve the quality of life and reduce the incidence of cardiovascular adverse events,which is worthy of clinical application.
