同步间歇指令通气治疗足月新生儿呼吸窘迫综合征疗效观察

目的探讨同步间歇指令通气（SIMV）治疗足月新生儿呼吸窘迫综合征（AHDS）的优越性及其临床价值。方法时36例需要机械通气治疗的足月新生儿ARDS患儿采用SIMV模式,观察上机后的临床表现、动脉血气变化及并发症的发生。结果36例实施SIMV后,25例呼吸困难、紫绀明显缓解或消失,pH值、动脉血氧分压（PaO2）及达到某一PaO2所需的吸氧浓度（FiO2）均较治疗前有所提高,而动脉血二氧化碳分压较治疗前有所下降。30例（83．3％）ARDS患儿临床治愈,4例死亡,2例自动放弃治疗。结论SIMV模式对新生儿ARDS治疗效果较好,具有临床推广价值。

Use of synchronized intermittent mandatory ventilation in the treatment of neonatal ARDS

Objective To review the experience of using synchronized intermittent mandatory ventilation in treatment of neonatal ARDS.Methods Synchronized intermittent mandatory ventilation(SIMV) was used to treated 36 neonates with ARDS ,clinical data including the clinical manifestation arterial blood-gas analysis and complications associated with ARDS were collected. Results Immediately after mechanical ventilation, dyspnea and cyanose of 25 patients alleviated or disappeared. Compared with pre-treatment, the pH, PaO2,PaO2/FiO2 were increased,and the PaCO2 was decreased.30 patients were cured,4 patients died,2 patients abandoned treatment. Conclusion SIMV has good effect on ARDS of neonate.
