基于真实世界研究益肾止衰颗粒对CKD 3~4期患者蛋白尿及尿足细胞的作用

目的:探讨益肾止衰颗粒对慢性肾脏病(CKD)3~4期患者蛋白尿及足细胞的作用。方法:基于真实世界自身对照的研究方法,纳入163例CKD3~4期患者,在原有治疗基础上加用益肾止衰颗粒,观察治疗12、24周的估算肾小球滤过率(eGFR)、24 h尿蛋白定量(24 h UTP)、尿足细胞标志蛋白podocalyxin(PCX)的变化。结果:163例患者经治疗后,eGFR逐步升高,24周与0周相比,升高有统计学意义(P<0.05)。24 h UTP逐渐下降,24周与0周相比,下降有统计学意义(P<0.05)。尿PTX逐渐降低,24周与0周相比,降低有统计学意义(P<0.05)。结论:益肾止衰颗粒对CKD3~4期患者具有一定的保护足细胞,减少尿足细胞排泄,降低蛋白尿的作用,从而保护肾功能。

Effect of Yishen Zhishuai Particles on Proteinuria and Podocytes in Patients with CKD of 3-4 Stage Based on Real-World Research

Objective:To explore the effect of Yishen Zhishuai Particles on proteinuria and podocytes in patients with chronic kidney disease(CKD)of 3-4 stage.Methods:Using the self-control method,163 patients with CKD of 3-4 stage were included.All patients were additional treated with Yishen Zhishuai Particles on the basis of the original treatment.Glomerular filtration rate(eGFR),24-h urinary total protein excretion(24 h UTP)and podocyte marker protein of podocalyxin(PCX)were observed after 12 and 24 weeks of the treatment.Results:eGFR was gradually increased(P<0.05),24 h UTP was gradually decreased(P<0.05),and urinary PTX also gradually decreased after 24 weeks of the treatment compared to those before the treatment in 163 patients(P<0.05).Conclusion:Yishen Zhishuai Particles has certain effects on protecting podocytes,reducing urinary podocyte excretion,and improving proteinuria in patients with CKD of 3-4 stage,thereby protecting renal function and delaying the progression of chronic renal failure.
