急诊电解可脱性弹簧圈血管内栓塞治疗颅内破裂动脉瘤

摘    要:目的探讨急诊电解可脱性弹簧圈(GDC)血管内栓塞颅内动脉瘤的临床疗效及应用价值。方法急诊用GDC血管内栓塞治疗颅内动脉瘤52例。结果成功栓塞52例55个颅内动脉瘤。51例痊愈,1例死亡。完全栓塞(>95%)32个,大部分栓塞(80%-95%)20个,部分栓塞(<79%)3个。并发动脉瘤破裂2例,其中1例痊愈,1例死亡。并发脑梗塞3例,局部溶栓后,遗留轻偏瘫和单瘫。2例弹簧圈末端遗留于动脉瘤外,经抗凝治疗未发生并发症。3例复发再次栓塞后痊愈。结论此方法是目前较为安全、可靠、有效的一种治疗方法。

Abstract:Objective To discuss the therapeutic effect and clinical value of emergency embolization of ruptured intracranial aneurysms with the Guglielmi detachable coils(GDCs). Methods 52 patients with 55 intracranial aneurysms were treated emergenfly with GDCs. Results 51 patients with 54 aneurysms were successfully embolized with GDCs. 32 cases achieved complete embolization (>95%), 20 with 80%-95% embolization and 3 with <79%. 2 aneurysms ruptured during the embolization, 1 cured and the other died. 3 patients developed cerebral infarction and had permanent slight hemiplegia or monoplegia. A small part of GDC was kept staying in the parent artery in 2 cases, but no complication after anticoagulant therapy. 3 recurring cases were cured by the second GDC embolization. Conclusion Emergency treatment of GDC embolization for intracranial aneurysms is so far a safe, reliable and effective method
