食管吻合器在右胸、上腹二切口手术治疗中段食管癌的应用

目的:探讨吻合器在右胸、上腹二切口手术治疗中段食管癌中的应用及疗效评价。方法:回顾性总结此类手术患者67例的手术经验。从术前评估、淋巴结的清扫、吻合口的缝合技术、术后并发症的预防方面探讨手术中应注意的问题。结果:全组无手术死亡。67例中1例术后24h发生ARDS,经呼吸机辅助呼吸等辅助治疗后治愈。无吻合口瘘,1例出现术后吻合口狭窄,经食管扩张术后缓解。结论:吻合器应用于右胸、上腹二切口手术治疗中段食管癌,具有吻合可靠、并发症少、淋巴结清扫彻底、缩短手术时间的优点。

Applications of stapler in abdominal and right thoracic approach for middle esophageal carcinaoma

Objective To evaluate the applications of stapler in abdominal and right thoracic approach for middle esophageal carcinoma.Method 67 patients were studied retrospectively.The factors involving preoperrative evaluation the suturing techniques,the preventation of postoperative complications were estimated.Results There was no death in this group.1 patient with ARDS was cured through ventilated machine.There were not fistula of anastomosed orifice,gastric and oesophageal countercurrent,1 patient had been recovered with anastomosed stricture by dilatation.Conclusion The use of stapler in abdominal and right thoracic approach for middle esophageal carcinaoma is effective and safe to improve lymphatic clearance and diminish the postoperative complications.
