成分输血在异位妊娠出血并弥漫性血管内凝血时的作用

目的观察并探讨成分输血在抢救异位妊娠出血弥漫性血管内凝血（DIC）时的作用。方法选取我院妇产科异位妊娠合并DIC产妇62例,随机分成对照组和治疗组各31例;对照组病人依需求输注红细胞悬液和血浆,治疗组病人采用成分输血,在输注红细胞悬液和血浆的基础上加用冷沉淀以及冷冻血小板。比较两组产妇输血后血色素（HGB）、红细胞比容（HCT）、血小板数（PLT）、凝血酶原时间（PT）、凝血酶时间（TT）、活化部分凝血活酶时间（APTT）以及纤维蛋白原（FGB）有无差别。结果抢救成功后,治疗组产妇HGB、HCT和TT检验指标与对照组差异无统计学意义（P〉0.05）。而治疗组在PLT、PT、APTT和FGB等方面的改善优于对照组（P〈0.05）。结论成分输血适用于异位妊娠大出血,尤其对改善大出血引起的DIC及相应并发症有重要作用。

Blood Component Therapy in Emergency Treatment of Ectopic Pregnancy Complicated with Disseminated Intravascular Coagulation

OBJECTIVE To observe the use of blood component therapy in gynecologic ectopic pregnancy complicated by disseminated intravascular coagulation（DIC）in the emergency. METHODS 62 patients with ectopic pregnancy complicated by DIC were randomly divided into two groups. Control group（31 cases） only used the red suspension and plasma, treatment group（31 cases） added with fresh frozen plasma, frozen platelets, cryoprecipitate. The hemoglobin（HGB）, hematocrit（HCT）, platelet count（PLT）, prothrombin time（PT）, thrombin time（TT）, activated partial clotting enzyme live time（APTT） and fibrinogen（FGB） of two groups were tested and compared after blood transfusion. RESULTS Treatment group and control group in the HGB, HCT, PT and TT testing had no significant difference（P〉0.05）, but the PLT, PT, APTT, and FGB of treatment group were better than control group（P〈0.05）.CONCLUSIONS Component transfusion is suitable for gynecologic ectopic pregnancy hemorrhage, especially plays an important role for improving of DIC caused by massive hemorrhage.
