苯磺酸氨氯地平联合缬沙坦对维持性血液透析高血压患者血压及蛋白尿的影响

目的探讨苯磺酸氨氯地平联合缬沙坦对维持性血液透析高血压患者血压及蛋白尿的影响。方法将108例维持性血液透析高血压患者随机分为对照组与观察组,每组54例。对照组患者给予苯磺酸氨氯地平口服,观察组患者接受苯磺酸氨氯地平联合缬沙坦口服,疗程均为3个月。比较两组治疗前后血压、尿蛋白及肾功能指标的改变。结果两组治疗后血压均较治疗前显著下降(P<0.05),但观察组降压幅度更显著(P<0.05);观察组治疗后血肌酐、尿素氮及24h尿蛋白定量显著少于对照组(P<0.05)。结论苯磺酸氨氯地平联合缬沙坦可显著降低维持性血液透析高血压患者的血压,并可减少尿蛋白及改善肾功能,具有肾脏保护作用。

Effect of Amlodipine Besylate Combined with Valsartan on Blood Pressure and Albuminuria in Maintenance Hemodialysis Patients with Hypertension

Objective To investigate the effect of amlodipine besylate combined with valsartan on blood pressure and albuminuria in maintenance hemodialysis patients with hypertension. Methods 108 maintenance hemodialysis patients with hypertension were randomly di- vided into the control group and the observation group,54 cases in each group. The control group was given oral amlodipine besylatethe,the observation group received oral amlodipine besylate combined with valsartan. The course of treatment in the two groups was 3 months. The changes of blood pressure, urinary protein and renal function indexes before and after treatment were compared between the two groups. Results After treatment,blood pressure in the two groups was significantly decreased than before treatment（P 〈 0. 05）,but the de- compression range of blood pressure in the observation group was significantly greater than that in the control group （P 〈 0.05）;and Seruin creatinine, BUN and 24-h urine protein quantity after treatment in the observation group were significantly less than those in the control group （P 〈 0. 05）. Conclusion Amlodipine besylate combined with valsartan can obviously decrease the blood pressure in the maintenance hemodialysis patients with hypertension,reduce urine protein,improve the renal function,and has a protective effect on the kidneys.
