进展期胃癌介入治疗的疗效评价

介入治疗进展期胃癌的方法主要有动脉灌注化疗，胃动脉栓塞术，经皮动脉穿刺植入药盒术等。对于可根治切除的进展期胃癌外科术前，术后及不能根治切除的进展期胃癌患者，可选择相应的方法，介入治疗术后疗效评价标准主要包括肿瘤病灶的改变，危及生命的临床症状的改善，肿瘤相关性抗原实验室检查，病理组织学，副反应，中位生存期，生存率的统计学分析和健康相关生命质量分析。目前的研究表明，进展期胃癌经介入治疗后确定起到了延长生存期并改善生存质量的效果，但是也还存在着一定的问题，今后的研究方向在于介入治疗方法的选择原则，如何最大限度减少对正常组织的损害以减少可能的并发症（尤其是痊塞剂的种类和量的研究），术前与术后介入治疗的选择原则，化疗方案的选择原则及循证医学在介入治疗进展期胃癌疗效评价中的应用。

Evaluation on the Effect of Interventional Treatment for Advanced Gastric Cancer

There are many methods of interventional treatment for advanced gastric cancer such as transcatheter arterial infusion,gastric arterial embolization,percutaneous port catheter system implantation,and so on.The criteria for evaluating the effect of interventional treatment for advanced gastric cancer includes the changing of cancer foci,ameliorating clinical symptoms,the laboratory examination of the tumor associated antigens,pathologic histology,side effects,medial survival,statistical analysis of survival and health related quality of life.The recent studies showed improving survival and quality of life with the patients after interventional therapy although there are still some problems.The direction of the study lies in the selection of interventional radiology for gastric cancer,decreasing the damage to healthy tissue,preoperative and postoperative intervention,selecting the protocol of chemotherapy and the application of evidence based medicine in interventional treatment for advanced gastric cancer.
