退行性心脏瓣膜病患者血清中MHC、IRF4、HSP70、HSP22mRNA表达的变化及意义

摘    要:目的 研究肌球蛋白重链(MHC)、干扰素调节因子-4(IRF-4)、热休克蛋白70(HSP70)、热休克蛋白22(HSP22)mRNA在老年退行性心脏瓣膜病(SDHVD)患者血清中的表达,探讨SDHVD的可能发病机制。方法根据彩色多普勒检查结果将所有入选者分为单纯主动脉瓣钙化组(AVC组,n=25)、单纯二尖瓣钙化组(MVC组,n=19)、主动脉瓣钙化合并二尖瓣钙化组(AVC+MVC组,n=21),24例心脏瓣膜无钙化的老年患者设为对照组(NVC)。应用逆转录聚合酶链反应(RT—PCR)方法检测入选者血清MHC、IRF-4、HSP70、HSP22mRNA的表达。结呆SDHVD组与对照组相比MHC和IRF-4mRNA表达量明显降低(P〈0.01),HSP70和HSP22mRNA表达量则明显增高(P〈0.01),不同瓣膜钙化损伤间无明显差异(P〉0.05)。SDHVD患者血清MHCmRNA、IRF-4mR—NA表达与LVEF呈正相关(r分别为0.394、0.427,P〈0.05),HSP70mRNA、HSP22mRNA表达量与LVEF呈负相关(r分别为-0.409、-0.422,P〈0.05)。结S-g-SDHVD患者血清MHC、IRF-4、HS1070、HSP22mRNA表达及心功能较心脏瓣膜无钙化者有明显变化,提示免疫炎性损伤参与SDHVD的发生发展。
