代谢综合征合并冠心病患者血浆P-选择素浓度的变化

目的：研究代谢综合征（MS）及合并冠心病（cAD）患者血浆P-选择素浓度的变化，探讨它与胰岛素抵抗（IR）的关系及其在MS发展为CAD中的作用。方法：80例患者，根据MS及CAD的诊断标准分为MS组（36例）和CAD组（44例），另选40例健康查体者作为正常对照组，抽取空腹静脉血，常规测量P-选择素。结果：血浆P-选择素浓度在正常对照组、MS组和MS＋CAD组中依次增高（25．t±3．6）ng／ml：（33．1±5．1）ng／ml：（41．3±8．4）ng／ml]，差异有显著性（P〈0．05），胰岛素敏感指数（ISI）在正常对照组显著高于其他二组（-1．5±0．1）：（-1.8±0．2）：（-1．8±8．4）]，差异有显著性（P〈0．05），血浆P-选择素水平与ISI呈负相关（r=-0．460，P〈0．05）。结论：P-选择素可能在代谢综合征发展为冠心病中起一定作用，并有潜在的临床应用价值。

Change of plasma P selectin levels in patients with metabolic syndrome and combined with coronary artery disease

Objective： To detect the change of the plasma P selectin levels in metabolic syndrome （MS） patients and in MS combined with coronary artery disease （CAD） patients, then to explore the relationship between P-selectin and insulin sensitivity index （ISI）, and the role of P-selectin in development from MS to CAD. Methods： A total of 80 MS patients were divided into two groups according to the diagnostic criteria of MS and CAD and 40 healthy subjects were randomly selected （normal control group）. Results： The plasma p-selectin levels increased one by one in normalcomrolgroup, MSgroup, MS＋ CAD group （25.1±3.6） ng/ml： （33.1±5.1） ng/ml： （41.3±8.4） ng/ ml, P〈0. 05]. The ISI in normal control group were much higher than that of other two groups （-1.5±0.1） vs. （-1.8±0.2） vs. （-1.8±8.4）, P〈0. 05]. The Pearson relation analysis showed that the plasma p--selectin levels were negatively correlated with ISI （r= 0. 460, P〈0.05）. Conclusion： P-selectin may play an important role in the develop from metabolic syndrome to coronary artery disease, and have potential clinical significance.
