大剂量奥施康定治疗31例重度癌痛疗效观察

目的观察大剂量奥施康定治疗重度癌痛的疗效及不良反应。方法奥施康定起始剂量30 mg/12 h,根据疼痛缓解程度进行剂量滴定。用药过程中疼痛未缓解、每日爆发痛次数超过2次,则增加下一次给药剂量,每次在原有基础上增加25%～50%,不增加给药次数。评价镇痛效果及不良反应。结果完全缓解(CR)8例(25.8%),部分缓解(PR)21例(67.7%),轻度缓解(MR)2例(6.5%),无效(NR)0例(0%),显效(CR+PR)29例(93.5%)。不良反应主要为便秘12例(38%),嗜睡5例(16.1%)。结论大剂量奥施康定治疗重度癌痛仍然安全有效。

The observation of curative effect in management of high-dose oxycontin on severe cancer pain of 31 cases

Aim To observe analgesic effect and main side effect of high-dose oxycontin in management of the severe cancer pain.Methods Oxycontin was administered at an initial dose of 30 mg every 12 h,the dose titration was according to the extent of pain relief,and only added dose by 25%~50% on the base of initial dose to the invalid or the patients of 2 times more paining every day,and evaluated analgesic effect and side effect.Results The invalid,minor,moderate and complete relief occurred in 0,2（6.5%）,21（67.7%）and 8（25.8%） patients,respectively,and pain relief provided by oxycontin was 93.5% for all patients combined.The adverse reactions were constipation 12（37.8%）,lethargy 5（16.1%）.Conclusion High-dose oxycontin was still safe and effective in the cure of severe cancer pain.
