激光治疗糖尿病性黄斑水肿疗效及机制探讨

目的　评价激光治疗糖尿病性黄斑水肿疗效 ,探讨糖尿病性黄斑水肿激光治疗的最佳时机。方法　对 186例 (2 32眼 )糖尿病性黄斑水肿患者激光疗效进行回顾性分析 ,随访时间 6个月。结果　局限性黄斑水肿者治疗有效率为 76 .0 9% ,弥漫性黄斑水肿者有效率为6 1.5 4 % ,黄斑囊样变性者有效率为最低仅 35 .17% (P <0 .0 1) ,差别有显著性意义。结论　糖尿病性黄斑水肿的分期有利于掌握治疗时机 ,早期进行激光治疗可以有效改善糖尿病性视网膜病变黄斑水肿患者的预后

Therapeutic effect and mechenism of photocoagulation for treatment of diabetic macular edema

Objective To evaluate the therapeutic efficacy of photocoagulation for diabetic macular edema(DME)and to determine the optimal timing of photocoagulation for DME.Methods Two handred and thirty two eyes (186 patients) with diabetic macular edema undergone photocoagulation were analyzed retrospectively; the patients were followed up for at least 6 months after the treatment.Results The visual acuity of 76.09%(35 of 46 eyes ) patients was improved in the localized DME group, 61.54%(48 of 78 eyes) improved in the diffuse DME group, and 35.17%( 32 of 91 eyes) improved in the cystoid DME group only. The resultant differences in visual acuity in these groups were significant statistically(P<0.01).Conclusion The awareness of the different stages of DME is helpful for the therapeutic efficacy, and early laser photocoagulation for diabetic macular edema is a more effective method in improving visual prognosis.
