冠心病与合并高血压病患者胰岛素抵抗的初步研究

对１７例冠心病（ＣＨＤ）和１５例高血压合并冠心病患者进行口服葡萄糖耐量试验，测定其血胰岛素（ＩＳ）和血糖水平，并与１０例健康人做对照。结果发现，ＣＨＤ和高血压合并冠心病患者都有胰岛素抵抗（ＩＲ），高胰岛素血症（ＨＩＳ）和部分糖耐量减低。高血压合并冠心病组ＩＲ和ＨＩＳ较ＣＨＤ组更为严重，且ＩＳ浓度呈持续增高状态。ＣＨＤ组血清总胆固醇与ＩＳ水平呈正相关，高血压合并冠心病组血清高密度脂蛋白胆固醇与ＩＳ水平呈显著负相关。

A PRELIMINARY STUDY ON INSULIN RESISTANCE IN CORONARY HEART DISEASE AND ESSENTIAL HYPERTENSION ASSOCIATED WITH CORONARY HEART DISEASE

Plasma glucose and insulin concentrations were measured(while fasting;one,two and three hours after a 75 g oral glucose load)in 17 patients with coronary heart disease(CHD),15 patients with essential hypertension associated with coronary heart disease(HC)and 10 healthy subjects.The results showed insulin resistance,hyperinsulinemia(plasma insulin>2 SD above the normal mean)and partial impaired glucose tolerance in all of the patients with CHD and HC,the degrees of insulin resistance and hyperinsulinemia were more marked in the latter than in the former.After an oral glucose load, plasma insulin concentrations at three hours were significantly higher in the HC than in the CHD.Plasma insulin levels positively correlated with total plasma cholesterol concentration in patients with CHD by multiple regression analysis,while plasma HDL-C and insulin levels revealed negative correlation in patients with HC.
