老年冠心病患者内皮依赖性舒张功能及颈动脉粥样硬化

目的：探讨老年冠心病（CHD）患者内皮依赖性舒张功能及颈动脉粥样硬化的状况。方法：选经冠状动脉造影证实为冠心病的老年患者96例，选冠状动脉造影正常，无糖尿病、脑血管病史20例老年人为对照组。两组进行了肱动脉内皮依赖性舒张功能及颈动脉超声的检测，颈动脉粥样硬化斑块积分采用Sutton法。结果：CHD内皮依赖性舒张功能比对照组明显降低（P〈0．01），CHD颈动脉内膜中层厚度（IMT），斑块积分显著高于对照组（P〈0．05）。斑块指数与冠状动脉狭窄程度呈正相关。结论：内皮细胞功能障碍与颈动脉粥样硬化和冠状动脉粥样硬化的病变相平行，内皮细胞功能障碍在粥样硬化的发病机制中起重要作用。

The Study On Endothelium Function and Carotid Atherosclerosis in Elderly Patients with Coronary Artery Disease

Objective: To investigate the endothelium dysfunction and carotid atherosclerosis and coronary atherosclerosis in elderly patients with coronary artery disease. Methods: The endothelium-dependent relaxing function of blood vessels, intimae - media thickness (IMT)of carotid arteries and the plague in carotid were measured using B - mode ultrasonography in 96 elderly coronary heart disease who had been examined by coronary angiography ,and 20 controls. Carotid plaque score was calculated using Sutton's method to quantify the extent of carotid atherosclerosis. Results: Endotelium dependent vascular diastolic-systolic function was significantly in CHD group versus controls. The carotid intimae - media thickness and plaque index were significantly increased in all diseased groups versus controls(p<0.05). Carotid plaque scores were positively correlated with coronary atherosclerosis. Conclusion: The result suggests that the endotheline disjunction and carotid atherosclerosis and coronary atherosclerosis are parallel endothelium dysfunction may play an important role in the pathology of CHD.
