超声测量房室平面位移评价左室舒张功能研究

摘    要:目的 探讨应用超声心动图测量房室平面位移 (AVPD)评价左心室舒张功能。方法 连续观察 92例受检者 ,其中正常者 19例 ,高血压患者 2 6例 ,冠心病 47例 (其中A E >13 2例 ;A E <110例 ;房颤 5例 ) ,心尖四腔切面用M型取样线置于二尖瓣环位置 ,AVPD随着M型移动曲线获得 ,测量房室平面位移最大值 (T)、左心房收缩引起房室平面位移值 (A) ;将脉冲多普勒取样容积置于二尖瓣口 ,测量舒张期血流频谱A E比值。结果 高血压病组冠心病组左心室房室平面全位移 (T)低于正常组 (P <0 .0 5 ;P <0 .0 0 1) ,左心房收缩引起房室平面位移及其与左心室房室平面全位移比值高于正常组 (P<0 .0 5 ;P <0 .0 0 1)。A T舒张功能参数与传统二尖瓣口录取A E比值存在线性相关 (r =0 .92 ;P <0 .0 0 1)。结论 上述超声心动图方法可用于评价左室舒张功能。

Abstract:Objective To determine the value of atrioventricular plane displacement (AVPD) in assessing left ventricular diastolic function using Doppler echocardiography. Methods Ninety two subjects,including 19 normal controls,47 with coronary artery disease(A/E>1 32 cases,A/E<1 10cases,Af 5 cases) and 26 patients with hypertension were examined consecutively.M mode echocardiographic studies were obtained from septal mitral annular sites:total AV plane displacement(T) and the values of AV plane displacement resulted from left atrial contraction (A) from apical 4 chamber views. Results The total AV plane displacement in diastolic stage were significantly lower in hypertensive and CAD subjects than in normal controls ( P <0.05; P <0.001 respectively).The valves of AV plane displacement resulted from left atrial contraction and its ratio were significantly higher in hyertension and CAD subjects than in healthy subjects ( P <0.05; P <0.001 respectively).Moreover,there was a close correlation between the A/T ration of the subjects examined and A/E rate by pulsed Doppler ( r =0.92; P <0.001). Conclusion Echocardiography AV plane displacement was useful in evaluation of left ventricular diastolic function.
