CEA、AFP、CA199、FERR联合检测对肝癌的诊断意义

摘    要:目的 探讨血清癌胚抗原(CEA)、甲胎蛋白(AFP)、糖类抗原199(CA199)和铁蛋白(FERRitin,FERR)联合检测在肝癌(LC)中的诊断意义。方法 选取2016-01~12在延安大学附属医院就诊的110例原发性肝癌患者(PHC),41例转移性肝癌患者(MHC),124例肝良性病变患者(良性组),及同期50例体检者(对照组),采用电化学发光技术检测PHC、MHC、良性组及对照组血清中的CEA、AFP、CA199、FERR的水平,并进行4组间单项及联合检测阳性率的比较。结果 MHC血清中CEA、CA199的水平最高,差异具有统计学意义(P<0.05);PHC组血清中AFP的水平最高,差异具有统计学意义(P<0.05);四项联合检测的阳性率在PHC与MHC差异无统计学意义(P>0.05);PHC与MHC四项联合测定阳性率均高于良性组及对照组,差异具有统计学意义(P<0.01);良性组四项联合测定的阳性率高于对照组,差异具有统计学意义(P<0.01)。结论 CEA、CA199对MHC诊断具有重要意义,AFP对PHC诊断具有重要意义,CEA、AFP、CA199、FERR四项联合检测可提高肝癌诊断的阳性率,有利于肝癌的早期诊断和治疗。

Abstract:Objective To investigate the diagnostic significance of combined detection of serum carcinoembryonic antigen (CEA),alpha-fetoprotein (AFP),carbohydrate antigen 199 (CA199) and ferritin (FERRitin,FERR) in liver cancer (LC).Methods A total of 110 patients with primary hepatocellular carcinoma cases (PHC),41 patients with metastatic hepatic carcinoma (MHC),124 patients with benign liver disease (benign group) and 50 medical examinations in the same period (control group) were enrolled in the Yan′an University Affiliated Hospital from January to December 2016.The levels of CEA,AFP,CA199 and FERR in serum of PHC,MHC,benign group and control group were detected by electrochemiluminescence technique,and the positive rates of single and combined detection among the four groups were compared.Results The levels of CEA and CA199 in MHC serum were the highest,the difference was statistically significant (P<0.05).The level of AFP in serum of PHC group was the highest,the difference was statistically significant (P<0.05).The positive rate of the four combined tests was not significantly different between PHC and MHC (P>0.05).The positive rate of PHC and MHC was higher than that of the benign group and the control group,the difference was statistically significant (P<0.01).The positive rate of the four combined tests in the benign group was higher than that in the control group,and the difference was statistically significant (P <0.01).Conclusion CEA and CA199 are important for the diagnosis of MHC.AFP is of great significance for the diagnosis of PHC.The combined detection of CEA,AFP,CA199 and FERR can improve the positive rate of liver cancer diagnosis and is beneficial to the early diagnosis and treatment of liver cancer.
