单颗前牙即刻种植的临床应用和牙龈美学评价

目的评价单颗前牙即刻种植修复后的牙龈美学效果。方法选择19例患者行即刻种植,同时使用异体脱细胞真皮基质行骨组织引导再生,观察种植体周软组织愈合情况,6个月后行组织环切二期手术及种植冠修复,分别于修复完成的当天及6、12个月观察种植修复体周软组织变化情况,并对其进行美学评价和满意度调查。结果种植成功率为100%,种植修复当天及6、12个月的牙龈附着水平测量值分别为(2.97±0.53)、(3.26±0.51)、(3.41±0.48)mm。28个龈乳头达到PIS 3级,10个为2级;改良菌斑指数为0或1,龈沟出血指数为0或1,牙龈色质正常,软组织健康状况好。84%的患者对种植义齿的美观效果满意,16%的患者表示基本满意,无不满意患者。结论龈乳头在修复完成后随着时间的推移有一定程度的自动恢复。应用即刻种植修复可达到良好的软组织美学效果,应用时一定要注意操作的技术要求。

Clinical and aesthetic evaluation on immediate implant of single maxillary anterior tooth

Objective To estimate the gingival aesthetics of immediate single-tooth implants in the anterior maxilla.Methods Nineteen patients,subjected to single-tooth replacements with immediate implant-supported restorations and treatments with ADM in the maxillary anterior region were included in the analysis.The delayed prosthetic restoration was placed approximately 6 months after implantation.The clinical and gingival aesthetical examinations were performed at peri-prosthesis,including papilla index score(PIS),variation of soft tissue around the implants,and questionnaire of satisfaction investigation.Res-ults The success rate of implants was 100%.The heights of gingival attachment level was detected at that very day,the sixth month and the 12th month after restoration,the results were(2.97±0.53) mm,(3.26±0.51) mm,(3.41±0.48) mm,respectively.Level 3 of the mesial and distal papillae index score,was observed in 28 cases,level 2 was attached in 10 cases respectively;gingival sulcus bleeding index was 0 or 1;the modified plaque index was 0 or 1.84% of patients were satisfied with the aesthetics.The basical satisfaction rate was 16%.Conclusion Gingival papillae would recover some extent.The immediate implant placement is a viable treatment option and promises an excellent esthetic solution for single maxillary anterior tooth,therefore,the strict technical requirement is the key.
