颈动脉体瘤的多层螺旋CT表现及诊断价值

摘    要:目的:探讨颈动脉体瘤的多层螺旋CT表现,评估其临床诊断价值。方法:搜集行多层螺旋CT检查,并经手术病理证实的颈动脉体瘤病例,利用原始的容积数据重建薄层图像,在工作站上采用多平面重组、最大密度投影及容积再现技术进行观察,分析颈动脉体瘤的形态特点、强化方式、与颈内外动脉的关系。结果:本组研究共纳入颈动脉体瘤病例16例,男性7例,女性9例。CT平扫瘤体呈等密度或稍高密度,增强后病灶明显强化,大部分强化欠均匀(14例),所有病灶的中心位置均位于颈总动脉分叉处,14例显示颈总动脉分叉处夹角扩大,其中7列瘤体较大,颈内外动脉完全受包绕,4例部分包绕颈内外动脉。多种后处理技术联合应用有助于观察病灶与邻近结构的解剖关系。结论:多层螺旋CT对颈动脉体瘤的诊断具有较大的价值,可作出准确的定位、定性诊断,而且有助于了解病灶与邻近结构的关系,对于治疗方案的制定具有重要意义。

Abstract:Objective:To assess the morphologic features and value of multidetector row helical CT(MDCT) on carotid body tumor.Methods:We retrospectively reviewed the MDCT data of patients with carotid body tumor proved surgically and pathologically.The thin slice images were transferred to a workstation and analyzed with multiplanar reconstruction(MPR),maxinum intensity projection(MIP) and volume rendering(VR) technique.Results:The study population consisted of 16 patients(man 7,women 9).All the tumors were visualized as masses of soft tissue and hyperenhanced after the injection of contrast agents with inhomegeneous enhancement in 14 patients.All the lesions were found to be located at region of the common carotid bifurcation accompanied with the enlarged angle.Both the internal and external carotid artery were totally encompassed in 7 patients and partially involved in 4 patients.The combined use of different postprocessing techniques provided a good display of the relationship between the tumor and neighboring structures.Conclusion:MDCT is helpful for the diagnosis of carotid body tumor and plays an important role in demonstrating the involvement of the neighboring structures,which can provide useful information for the treatment.
