分期微创治疗膝关节多韧带损伤的疗效观察

目的：探讨分期微创治疗膝关节多韧带损伤的临床效果。方法2009年1月至2010年5月于广州军区广州总医院采用分期微创法治疗20例膝关节脱位多韧带损伤患者，根据Lysholm和IKDC 2000评分标准进行疗效评价。结果20例患者平均随访15个月（12～36个月），Lysholm膝关节功能评分由术前的（26±5）分提高到术后6个月的（94±5）分（t=44.270，P=0.000）；IKDC 2000评分由术前的D级20例提高到术后6个月的A级12例、B级6例、C级2例。结论严格把握治疗时机、分期对关节内外韧带进行修复重建有利于关节功能的恢复，治疗膝关节多韧带损伤效果好。

Clinical effect of minimally invasive surgical treatment for multiple ligament injury by stages

Objective To explore the effect of minimally invasive surgical treatment for multiple ligament injury by stages. Methods From January 2009 to May 2010, 20 patients with multiple ligament injuries were treated by stages in Guangzhou General Hospital of Guangzhou Military Command. Therapeutic effects were evaluated by Lysholm knee function score and IKDC 2000 score. Results All patients were followed up for an average of 15 months （12-36 months）. Lysholm knee functional score was improved from preoperative 26 ± 5 to postoperative 94 ± 5 （t = 44.270,P = 0.000）;According to IKDC 2000 scoring, there were A grade in 12 cases, B grade in 6 cases and C grade in 2 cases at 6 month＆#39;s follow-up, while there was D grade in all 20 patients before the operation. Conclusion With good understanding of surgical timing, ligament repair and reconstruction by stages could achieve good efficacy for multiple ligament injury because it is beneficial to the recovery of joint function.
