不同类型HPV与宫颈上皮内瘤变和宫颈癌的关系

目的探讨宫颈上皮内瘤变(CIN)和宫颈癌中不同类型人乳头瘤病毒(HR-HPV)的感染状况。方法回顾性研究妇科门诊2520例已确诊的CIN和宫颈癌患者,采用聚合酶链反应(PCR)技术进行HPV分型检测,和第2代杂交捕获试验法(HybridCaptureII,HCII)检测高危型HPV,对比低危型HPV6/11和高危型HPV16、18两组型别感染者年龄和宫颈病变程度。结果 (1)2520例女性宫颈拭子HPV总感染1786例,阳性率70.87%;高危型HPV16、181404例,阳性率55.71%;低危型HPV6/11756例,阳性率30%。高危型HPV感染率以宫颈癌组的最高64.29%(P0.005)。(2)在CINI和CINII～III患者的所有年龄组中,20～35岁所占比例最高,分别为50.78%和40.26%;在宫颈癌患者中,35～50岁所占比例最高38.25%。(3)HPV感染的高峰年龄为35岁,HPV阳性率达87.66%(P0.005),并随年龄的增长,阳性率呈下降趋势;低危型HPV6/11感染的高峰年龄亦35岁,占阳性例数的55.68%(P0.005);高危型HPV16感染的高峰年龄为50～65岁,占阳性例数的12.9%;高危型HPV18感染的高峰年龄为65岁,占阳性例数的40.20%(P0.01)。低危型HPV6/11多见于年轻妇女,而高危型HPV16、18则多见于中老年妇女。(4)在CINI组中,低危型HPV6/11所占阳性例数的比例最高82.42%。在CINII～III及宫颈癌组中,均以高危型HPV16所占阳性例数的比例最高,分别为37.46%、42%。结论女性生殖道HPV感染率随宫颈病变程度的加重呈现升高趋势;不同宫颈病变中HPV型别分布不同。
