老年冠心病患者认知功能与胰岛素抵抗关系的研究

目的：探讨老年冠心病患者认知功能与胰岛素抵抗（IR）的关系。方法：对148例老年冠心病患者分为两组：合并认知功能损害患者组62例,认知功能正常患者组86例,比较两组间性别、年龄、文化程度、BMI、WHR、TC、TG、CRP、HDL-C、LDL-C、HOMA-IR、SBP和DBP的差异,并进行Logistic回归分析。结果：与认知功能正常组相比,老年冠心病合并认知功能损害组文化程度较低,BMI、WHR、TC、TG、FPG、HDL-C、LDL-C、CRP、SBP、DBP及HOMA-IR较高（P〈0.05）;Logistic回归分析显示文化程度、BMI、WHR及HOMA-IR对认知功能障碍的发病具有影响。结论：低文化程度、BMI、WHR及HOMA-IR是认知功能障碍的独立危险因素。

Relationship between cognitive impairment and insulin resistance in elderly patients with Coronary heart disease

Objective ：To explore the relationship between cognitive impairment and insulin resistance in elderly patients with Coronary heart disease. Methods： One hundred and forty-eight patients were divided into two groups：cognitive im- pairment group and non-cognitive impairment group. The height, weight, waist circumference, hip circumference, body mass index （BMI）, waist to hip ratio （WHR）, fasting serum glucose, insulin, total cholesterol （TC）, triglyceride （TG）, and C-RP were detected. The difference of these indexes and HOMA-IR were compared between cognitive impairment group and non-cognitive impairment group. Results： Compared with non-cognitive impairment group, the BMI, WHR, FPG, HDL-C ,LDL-C, CRP, SBP,DBP and HOMA-IR in cognitive impairment group increased markedly （P 〈 0.05 ）, cul- tural level was lower. The Logistic regression analysis showed that cultural level, HOMA-IR, BMI and WHR had an im- pact on the incidence of NAFLD. Conclusion： HOMA-IR, cultural level, BMI and WHR were the independent risk factors for developing cognitive impairment in elderly patients with Coronary heart disease.
