Penn 尿囊术11例报告

目的：探讨Penn尿囊术的临床疗效。方法：对11例膀胱癌患者行全膀胱切除加Penn尿囊术后的尿囊容量、压力,可控程度及并发症进行观察。结果：11例均应得随访,平均随访时间22．5个月(12～42个月),术后3个月尿囊的平均最大容量为482ml,囊内压1．88kPa。术后1年,尿囊平均容量增至593ml,而囊内压降至1．25kPa,全部病例尿控满意,无输尿管尿液反流及吻合狭窄和明显代谢紊乱。阑尾输出口狭窄2例,尿囊结石1例。结论：Penn尿囊具有容量大,内压低,以及较好的顺应性,自控排尿良好,是一种较为满意的可控性尿流改道术。

Penn pouch (Report of 11 cases)

Purpose:To evaluate the use of Penn Pouch for continent urinary diversion. Methods:A technique of Penn pouch has been performed in 11 patients following total cystectomy. Therapeutic effects (continence, intrareservoir pressure and capacity) and complications were observed. Results:All the patients have been followed up for a mean of 22.5 months ranging from 12 to 42 months. After 12 months the average maximum capacity of reservoir was 482 ml 3 months postoperatively and mounted up 593 ml, the average maximum pressure were 1.88 kPa 3 months postoperatively and down to 1.25 kPa. All patients got continence. Neither ureter reflux nor anastomotic stoma stricture nor metabolic disturbance has been observed. Appendicular stoma stenosis and reservoir stone occurred in 2 and 1 respectively. Conclusions:The Penn pouch possessed high capacity, low tension, good compliance and excellent urinary continence . Therefore, Penn pouch is satisfactory means of continent urinary diversions.
