手术与药物联合治疗难治性胃溃疡的疗效观察

目的：观察采用不同的手术方法与抗幽门螺杆菌药物治疗难治性胃溃疡的疗效。方法：选择我科2008年5月～2010年12月应用手术联合抗幽门螺杆菌药物治疗的难治性胃溃疡患者45例为观察组，同期行药物保守治疗的45例患者为对照组，观察组接受手术联合抗幽门螺杆菌抗生素治疗，对照组只采用药物治疗。结果：两组疗效、幽门螺杆菌根除率、生活质量评分比较，差异有统计学意义（P〈0．05），两组患者出血、穿孔、感染、狭窄等并发症发生率比较，除感染外，差异均有统计学意义（P〈0．05），观察组的疗效优于对照组。结论：手术联合抗幽门螺杆菌治疗难治性胃溃疡效果好，能显著提高溃疡治愈率和幽门螺杆菌根除率，降低复发率和并发症发生率，从根本上提高患者的生活质量。

The observation of effect of the anti-pylorus helicobacter and operation on difficult gastric ulcer

Objective To observe the effect of operation combined with surgical treatment of refractory helicobacter pylori gastric ulcer.Meth- ods 90 patients with refractory gastric ulcer patients were divided into observation and control groups.Observation group were treated with surgery and antibiotic treatment against helicobacter pylori, control group treated only with drugs.Resuhs After treatment,there were signifi- cant difference in the cure rate of gastric ulcer,recurrence rate,helicobacter pylori eradication rate,complication rate and quality of life score be- tween observation group and control group （P~O.05）.Conclusion Surgery combined with anti-helicobacter pylori treatment of refractory gastric ulcer, can significantly improve the cure rate and Hp eradication rate,lower recurrence rate and complication rate and improves the quality of life.
