围手术期预防性应用抗菌药物分析

目的调查分析和评价某医院外科手术患者围手术期预防性使用抗菌药物情况。方法采用系统抽样方法 ,抽取某医院2009年出院的外科手术患者的住院病历158份,对围手术期抗菌药物使用情况进行回顾性调查分析。结果 158例手术患者预防性抗菌药物使用率100.00%,用药时间1~27 d,平均(5.86±3.43)d;用药时机:术前2 h用药占77.22%,22.78%的病例抗菌药物用药时机不合理。结论根据卫办医发[2008]48号《通知》,158例围手术期预防性应用抗菌药物存在用药指征把握不严、抗菌药物种类选择欠妥、盲目联合用药、用药时机不当和用药时间长等问题,亟需加强规范化管理。

Clinical Usage of Prophylactic Antibacterials in Surgical Patients

OBJECTIVE To survey the clinical usage of prophylactic antibacterials in surgical patients in a hospital.METHODS The use of antibacterials in 158 surgical patients undergoing operations in a hospital from Jan to Jun 2009 was reviewed.RESULTS All of patients received prophylactic antibacterials.Among them 50 patients (31.65％) received one kind of antibacterial and 108 patients (68.35％) received≥2 kinds of antibacterials.The average duration of antibacterial use was (5.86±3.43) d.CONCLUSIONS There are some problems in prophylactic antibacterial usage,such as the using indication and occasion are not appropriate,and some antibacterial agents are selected unreasonably Or used excessively.It is urgent to strengthen the management of antibacterial usage.
