万古霉素血药浓度监测的临床应用

摘    要:
目的 探讨万古霉素血药浓度监测的临床应用,以指导临床合理用药.方法 收集临床上有病原学依据而应用万古霉素病人的血液标本,应用高效液相色谱法(HPLC)测定万古霉素血药浓度,实现个体化给药.结果 18例病人进行万古霉素血药浓度监测,设为治疗组,其中4例病人血药浓度在治疗窗范围内,未调整给药方案;14例病人经调整给药后,血药浓度亦控制在正常范围内.34例病人未进行万古霉素血药浓度测定,设为对照组.治疗组与对照组病人治愈率分别为100%和76.5%,两组疗效差异有统计学意义(P<0.05);肾毒性发生率分别为0和26.5%,两组肾毒性差异有统计学意义(P<0.05);两组均未出现耳毒性病人.结论 应用高效液相色谱法测定万古霉素血药浓度,根据血药浓度制定个体化给药方案,能提高疗效,减少不良反应.
