影响颈椎病后路手术疗效因素分析

目的 分析脊髓型颈椎病后路手术疗效不佳原因。方法 总结15例脊髓型颈椎病后路手术取椎板减压，颈椎管成形术疗效不佳病例，找出影响疗效主要因素。结果 15例中术前诊断失误3例，术前脊髓萎缩或软化3例，合并颈椎畸形3例，术中定位错误减压不充分3例，术中损伤脊髓2例，术后并发神经麻痹1例。结论 后路手术适应症、颈椎畸形、脊髓萎缩软化、减压不充分、术中损伤脊髓，都是影响手术疗效的因素。

An analysis of the factors affecting the Results of treatment of cervical myelopathy through posterior surgical approach.

Objective:To analyze the factors affecting the results of treatment of cervical mylopathy through a posterior surgical approach.Results:In this series of 15 cases of cervical spondylopathy,3 were accompanied with preoperative misdiagnosis,3 with atrophy or softening of the spinal cord.3 with cervical deformity,3 with incomplete decompression due to incorrect orientation during the operation,2 with intraoperative injury of spind cord,1 with cervical nerve paralysis after operation.Conclusion:The right selection of the posterior surgical approach,cervical deformity atrophy or softening of spinal cord,incomplete decompression and intraoperative injury of spinal cord are factors affecting the final outcome.
