耳穴贴压联合针灸辅助激素治疗月经不调临床研究

目的:研究耳穴贴压联合针灸辅助激素治疗月经不调的临床疗效及其作用机制。方法:将我院妇科门诊收治的月经不调患者125例随机分为两组,其中观察组63例采用耳穴贴压联合针灸治疗,对照组62例给予戊酸雌二醇片和黄体酮软胶囊口服,治疗时间均为6个月,比较两组有效率、排卵率、性激素水平、子宫内膜厚度及不良反应。结果:观察组和对照组有效率分别为93.65%和80.65%,两组治疗效果差异有统计学意义(P<0.05)。观察组月经恢复时间明显低于对照组(P<0.05),排卵率明显高于对照组(P<0.05)。治疗后两组血清FSH和LH水平明显降低(P<0.05),E₂和P水平明显升高(P<0.05),且观察组血清FSH和LH水平低于对照组,E₂和P水平高于对照组。两组子宫内膜厚度均明显升高(P<0.05),且观察组子宫内膜厚度高于对照组(P<0.05)。观察组治疗期间发生头晕1例、恶心2例,合计3例(4.76%),对照组发生头晕2例(3.23%)、恶心4例(6.45%)、腹痛1例(1.61%),合计7例(11.29%),两组不良反应比较差异无统计学意义(χ²=1.810,P=0.179)。结论:耳穴贴压联合针灸辅助激素治疗月经不调具有良好效果,有利于改善患者性激素水平,促进卵泡和子宫内膜发育,从而使月经周期恢复正常。

Clinical efficacy of auricular point sticking combined with acupuncture and moxibustion assisted with hormones on irregular menstruation

Objective:To study the clinical efficacy and role mechanism of auricular point sticking combined with acupuncture and moxibustion assisted with hormones in the treatment of irregular menstruation.Methods:A total of 125 patients with irregular menstruation admitted to gynecological clinic of our hospital were randomly divided into two groups.63 patients in observation group were treated with auricular point sticking combined with acupuncture and moxibustion.62 patients in control group were given estradiol valerate tablets and Progesterone soft capsule orally,and they were treated for 6 months.The effective rate,ovulation rate,sex hormones levels,endometrial thickness and adverse reactions were compared between the two groups.Results:The effective rates in the observation group and the control group were 93.65% and 80.65% respectively(P<0.05),and there were statistically significant differences in the treatment effects between the two groups(P<0.05).The menstrual recovery time in observation group was significantly lower than that in control group(P<0.05)while the ovulation rate was significantly higher than that in the control group(P<0.05).After treatment,the levels of serum FSH and LH in the two groups were significantly decreased(P<0.05)while the levels of E₂ and P were significantly increased(P<0.05),and the levels of serum FSH and LH in observation group were lower than those in the control group while the levels of E₂ and P were higher than those in the control group.The endometrial thickness was significantly increased in the two groups(P<0.05),and the endometrial thickness in the observation group was higher than that in the control group(P<0.05).There was 1 case of dizziness and 2 cases of nausea in observation group,3 cases(4.76%)in total,and there were 2 cases(3.23%)of dizziness,4 cases(6.45%)of nausea,1 case(1.61%)of abdominal pain in control group,7 cases(11.29%)in total.There were no significant difference in the adverse reactions between the two groups(χ²=1.810,P=0.179).Conclusion: Auricular point sticking combined with acupuncture and moxibustion assisted with hormones has good effect on irregular menstruation,which is beneficial to improve the levels of sex hormones and promote thedevelopment of follicles and endometrium,thus restoring the menstrual cycle.
