颅颌面联合手术治疗前颅底区肿瘤

目的 探讨前颅底肿瘤手术及术后颅底组织缺损修复方法。方法 对１９９２～１９９９年间我院收治的３５例患者的手术方法、手术效果、并发症、生存率、修复材料和方法等进行了回顾性分析。结果 本组并发症的发生率为１４．２９％（５／３５例），３５例中良性肿瘤共１１例，１例死于术后脑水肿伴全身，余了０例随访７年皆健在。２４例恶性肿瘤患者的３年和５年生存率（按生命表法计算）分别为５８．０７％和３６．２９％。结论 采

A combined craniofacial approach to surgical treatment of tumors involving the anterior skull base

Objective To sum up experience in surgical treatment of tumors in the anterior skull base and address the method for reconstruction of the skull base defect thereof .Methods We retrospectively reviewed the medical charts of 35 patients with malignant tumors involving the anterior skull base who underwent combined craniofacial resection in our hospital from, 1992 to 1999.The data on histological findings,extent of surgical procedure,type of reconstruction,and follow up results were analyzed.Results twenty three patients (65.7%) had extensive lesions directly invading the anterior skull base;6 patients(17.14%)could not have the eyeball preserved;31 patients(88.57%) underwent microsurgical reconstruction for the anterior skull base defect.The postoperative complications occurred in 5 patients(14.29%).The 5 year and 3 year survival rates were 36.29% and 58.07% respectively.Conclusion This data comfirms that the anterior skull base tumors were resected by the combined craniofacial resection and microsurgical reconstruction is satisfactory.
