基于乡镇卫生院患方研究的医患信任现状分析

目的：在了解乡镇卫生院患方信任现状及其影响因素的基础上，提出构建医患信任关系的对策和建议。方法：对北京市北部某远郊区3个乡镇卫生院进行为期9天的实地观察，并对24位患者进行定性访谈，分析患方对医方信任情况及其影响因素。结果：患者对城市大医院医生的总体信任评分高于乡镇卫生院，其主要影响因素是乡镇卫生院医生技术水平较低、缺少适应患者需求的设备和药品、公共卫生不能满足患者需求等。乡镇卫生院的患方信任类型为声誉型信任，影响其信任的因素主要为诊疗技术、医患沟通和报销制度等方面。结论：乡镇卫生院可通过提升医生诊疗技术水平，加强公共卫生人才队伍建设，增加适应患者需求的设备和药品种类等方法，以提高患者对医方的信任度，从而建立和谐的医患关系。

Study of Doctor - patient Trust in Township Hospitals in the View of Patients

Objective ： Describing the trust status of township hospitals and influencing factors, then making relevant suggestions. Methods ： There is an observation period of nine days for three township hospitals in suburban north of Beijing with interviewing 24 qualitative patients, describing and analyzing the status of trust and its influencing factors. Results： Township hospitals trust was inferior to large urban hospitals, mainly due to the lack of technology. On the other hand, township hospitals lack of the necessary equipment and drug which patients need, and the pub- lic health cant meet the patient demand, which influence the trust about providing service. The type of patients trust in township hospitals is repu- tation - based trust ; trust affecting factors is medical technology, patient communication and reimbursement system. Conclusion： The patient trust of township hospitals can be improved by increasing treatment technology, strengthening the public health talent team construction, improving the equipment configuration levels and types of drugs increased.
