摘    要:
目的 分析医院内获得性肺炎(hospital acquired pneumonia,HAP)培养病原菌在各病区的分布和细菌耐药率.方法 对我院京西院区2年来ICU、内科、外科病房HAP患者培养病原菌阳性标本进行分析,比较各病区间的异同,分析病原菌的药物敏感试验结果.结果 全院共获阳性标本1183份.铜绿假单胞菌、肺炎克雷伯杆菌、鲍氏不动杆菌、金黄色葡萄球菌、大肠埃希菌占各病区菌种的前5位.铜绿假单胞菌和金黄色葡萄球菌在ICU的构成比明显高于其他病区.大肠埃希菌在外科的构成比明显高于其他病区.革兰阴性杆菌的耐药率普遍较高.各病区分离的金黄色葡萄球菌均对苯唑西林高度耐药.结论 HAP病原菌谱在各病区间具有不同分布;病原菌耐药性普遍较高.

Abstract:
Objective To study the distribution and drug resistance of hospital acquired pneumonia (HAP) pathogens in different wards. Methods To analyse the results of sputum culture of HAP patients in ICU, medieal wards and surgery wards in 2 years. Results 1183 strain of bacteria were isolated.Pseudomonas aeruginosa, Klebsiella pneumoniae, Acinetobacter baumannii, Staphylococcus aureus and Escherichia coli were the major strains in all wards. The constituent ratio of Pseudomonas aeruginosa and Staphylococcus aureus in ICU was higher than .that in medical wards and surgery wards. The constituent ratio of Escherichia coli in surgery wards was higher than that in ICU and medical wards. The gram negative bacteria had high drug resistance. Staphylococcus aureus were oxacillin resistant in all wards. Conclusions The pathogen spectrum of HAP is different in ICU,medical wards and surgery wards. The pathogen had high drug resistance.
