尿毒症维持性血液透析患者C-反应蛋白变化及临床意义

摘    要:
目的 探讨尿毒症维持性血液透析患者C -反应蛋白的改变及相关临床意义。方法 抽取 2 1例尿毒症维持性血液透析患者血液透析前和血液透析后的C -反应蛋白 (CRP)、血肌酐 (SCr)、血清白蛋白(ALB)、血红蛋白 (Hb)及 18例健康者作为对照组。结果 尿毒症维持性血液透析患者透析前CRP正常与异常组之间ALB、Hb差异有显著性 (P <0 .0 5 ) ,透析后CRP水平升高、血液透析前与血液透析后比较差异有显著性 (P <0 .0 5 ) ,且血液透析后患者CRP与其ALB、Hb显著负相关 (P <0 .0 5 )。结论 CRP升高发生炎症反应在维持性血液透析患者营养不良、贫血中起重要作用 ,且CRP升高发生炎症反应与血液透析本身的因素及个体因素如感染、营养状况等相关。

Abstract:
Objective To study the clinical significance of chan ges of C-reative protein(CRP). Methods Their CRP,Serum creatinine(SCr), albumin(ALB), Hemoglobi n(Hb) were measured twenty-one maintenance hemodialysis patients and 18 normal c ontrols were investigated. Results There were significant difference (P<0.05) of serum AlB and Hb between normal CRP patients and abnormal CRP patients before CRP patients before hemodialysis. The level of serum CRP increased after hemodialysis. The difference of CRP level before and after hemodialysis was significant (P<0.05). Negative correlation was found between the levels of CRP and ALB and Hb after hemodialysis (P<0.05).Conclusion The increasing CRP levels resulted in inflammatory st ate of uremia patients and that played a role in the occurance of malnutrition a nd anemia of maintenance hemodialysis patients. The increasing of CRP level and inflammatory reaction are correlated with some factors of hemodialysis and patie nts such as infection, nutrtional status etc.
