慢性阻塞性肺疾病患者血二氧化碳分压与血浆纤维蛋白原的相关性研究

摘    要:
目的探讨慢性阻塞性肺疾病(COPD)患者血浆纤维蛋白原水平及其与血二氧化碳分压的相关性。方法对30例健康者及30例COPD患者治疗前后血二氧化碳分压、血浆纤维蛋白原进行测定。结果COPD发作期患者血浆纤维蛋白原水平明显升高,治疗后明显下降,血浆纤维蛋白原与血二氧化碳分压呈正相关(r=0.75,P<0.01)。结论COPD患者血中纤维蛋白原含量异常增加可能造成血液的高凝状态和肺小动脉血栓的形成,高碳酸血症与血浆纤维蛋白原增高有密切相关性。改善通气及适当的抗凝治疗对控制COPD病情可能有益。

Abstract:
Objective It is to discuss the plasma fibrinogen level and the correlation with p(CO_2) in patients with chronic obstructive pulmonary disease (COPD). Methods p(CO_2) and plasma fibrinogen level of 30 healthy people and 30 patients with COPD before and after treatment were measured. Results The plasma fibrinogen levels of patients with COPD rose obviously in onset period and dropped obviously after treatment. Plasma fibrinogen was positively correlated with p(CO_2) (r=0.75,P<0.01). Conclusion Abnormal increase of fibrinogen in blood of patients with COPD may lead to blood hypercoagulability state and the formulation of small pulmonary artery thrombus. There is a close correlation between hypercapnia and the increase of plasma fibrinogen. Improving ventilation and proper anti-coagulation treatment may be benefit to control the pathogenetic condition of COPD.
