卵巢肿瘤蒂扭转腹腔镜下复位保留卵巢手术临床研究

目的总结32例卵巢肿瘤蒂扭转患者的临床资料和腹腔镜下复位保留卵巢手术处理结果,探讨卵巢肿瘤蒂扭转腹腔镜下保留卵巢手术的可行性。方法32例患者,均以腹部疼痛就诊,均进行妇科检查及超声检查,并及时行腹腔镜探查术,术中先将扭转的肿瘤复位,再根据卵巢组织缺血情况,分别给予温盐水冲洗、浸泡,切开吸出凝血块等处理后剥离肿瘤并送快速病理检查。结果32例患者均证实为卵巢良性肿瘤蒂扭转,均顺利完成手术,平均手术时间50±23min,平均出血量30±16ml,术后无1例发生栓塞。10例患者随访7～36个月,其中9例卵巢功能恢复正常。结论卵巢肿瘤蒂扭转早期诊断,及时行腹腔镜下保留卵巢功能的手术是安全有效的治疗方法。
