经阴道与腹腔镜肿瘤切除手术对卵巢良性肿瘤患者阴道菌群及卵巢功能的影响

目的 探讨经阴道与腹腔镜肿瘤切除手术对卵巢良性肿瘤患者阴道菌群及卵巢功能的影响.方法 选取2013年5月至2014年9月本院收治的卵巢良性肿瘤患者76例为研究对象,采用随机数表法分为观察组和对照组,各38例;对照组经阴道实施肿瘤切除术,观察组在腹腔镜下实施肿瘤切除术,比较两组术后阴道菌群失调率,分析其白细胞(WBC)与C反应蛋白(CRP)水平,并测定两组血清雌二醇(E2)、卵泡激素(FSH)、黄体生成素(LH)水平,同时记录并发症.结果 手术后观察组阴道菌群失调率为42.1％、并发症发生率为5.3％,明显低于对照组的68.4％、21.1％(均P＜0.05);术后两组E2、FSH、LH水平均下降(均P＜ 0.05),观察组E2为(0.10±0.02) nmol/L、FSH为(10.40±2.63) U/L、LH为(7.50±1.87) U/L,与对照组比较明显较高(均P＜ 0.05);观察组WBC为(8.51±1.23) 109/L、CRP为(11.09±4.35) mg/L,低于对照组(均P＜0.05).结论 与经阴道手术相比,腹腔镜肿瘤切除手术对卵巢良性肿瘤患者阴道菌群及卵巢功能影响小,且并发症发生率低,值得在临床推广应用.

Effects of transvaginal versus laparoscopic tumor excision on vaginal flora and ovarian function in patients with ovarian benign tumor

Objective To explore effects oftransvaginal versus laparoscopic tumor excision on vaginal flora and ovarian function in patients with ovarian benign tumor.Methods 76 patients with benign ovarian benign tumor treated at our hospital from May,2013 to September,2014 were selected as study objects and were divided into an observation group and a control group by random number table,38 cases for each group.The control group received transvaginal tumor excision and the observation group laparoscopic tumor excision.The postoperative vaginal flora dysfunction rates in the two groups were compared.The levels of white blood cell (WBC) and C-reactive protein (CRP) were analyzed.The serum levels of estradiol (E2),follicular hormone (FSH),and luteinizing hormone (LH) in the two groups were determined.The complications were recorded.Results The postoperative vaginal flora dysfunction rate and the incidence of complications were 42.1％ and 5.3％ in the observation group and were 68.4％ and 21.1％ the control group (P ＜ 0.05).The levels ofE2,FSH,and LH were after than before operation in both groups (P ＜ 0.05) and were (0.10±0.02) nmol/L,(10.40±2.63)U/L,and (7.50±1.87)U/L in the observation group after operation,which were significantly higher than those in the control group (P ＜ 0.05).The levels of WBC and CRP were (8.51±1.23)109/L and (11.09±4.35) mg/L in the observation group,which were significantly lower than those the control group (P ＜ 0.05).Conclusions Compared with transvaginal surgery,the effect of laparoscopic tumor excision on vaginal flora and ovarian function in patients with ovarian benign tumor is smaller and has a lower incidence of complications,so it is worth being clinically generalized.
