初始耐药对肺结核治疗的影响

目的了解结核菌初始耐药对肺结核治疗的影响。方法对我院2002年1月~2004年12月有结核菌培养及药敏结果以及门诊随访记录资料完整的初治肺结核患者109例,随机分为敏感组（62例）、耐药组（47例）进行分析比较。结果敏感组和耐药组患者的痰菌阴转率2个月末分别为71.0%、51.1%（P〈0.05）,9个月末分别为93.5%、74.5%（P〈0.01）;9个月末疗程结束时,耐单药组、耐二药及以上非MDR-TB组和耐多药（MDR-TB）组痰菌阴转率分别为88.5%、61.5%和50.0%（P〈0.01）。9个月末疗程结束时,敏感组和耐药组患者的X线吸收好转率分别为90.3%、72.3%（P〈0.05）,空洞闭合率分别为89.7%、71.0%（P〈0.05）。敏感组和耐药组患者的治疗成功率分别为91.9%、70.2%（P〈0.01）,2年内复发率分别为1.8%、15.1%（P〈0.05）。结论初始耐药是初治肺结核治疗失败的主要原因。
