血清游离脂肪酸与2型糖尿病患者胰岛素抵抗的相关性研究

摘    要:目的探讨空腹及餐后2h血清游离脂肪酸与2型糖尿病(T2DM)患者胰岛素抵抗的相关性。方法共收集T2DM患者85例,按照HbA1c检测值将其分为控制不良组(n=30)、控制一般或者良好组(n=55)。选取同期来我院进行健康体检的健康研究对象38例作为正常对照组。采用酶电极法对葡萄糖进行测定,酶联法对FFA进行测定,化学发光法对胰岛素进行测定,采用高效液相法对HbA1c进行测定,HOMA—IR用稳态模式评估。结果T2DM组与正常对照组比较HbA1c、FPG、2hPG、HOMA—IR、FFA及2hFFA各项指标的检测值均明显较高,差异具有统计学意义(P〈0.05)。与T2DM组中的控制良好组比较,控制不良组患者的HbAIc、FPG、2hPG、HOMA—IR、FFA及2hFFA各项指标均明显较高,差异有统计学意义(P〈0.05)。FFA、2hFFA明显正相关于HbA1c、FPG、2hPG、HOMA—IR(P〈0.05)。结论T2DM患者体内FFA水平的升高与胰岛素抵抗具有密切的关系,在T2DM的发病机制中具有重要的作用。

Abstract:Objective To explore the correlation between fasting and 2h postprandial serum free fatty acid and insulin resistance in patients with type 2 diabetes. Methods This study collected 85 T2DM patients, they were divided into the control group (n=30) and control group or good (n=55) according to the HbA1c detection value. We selected 38 cases of healthy on the same period as the T2DM patients as normal control group. The determination of glucose were determined with the enzyme electrode method, ELISA method was used to determine FFA, chemiluminescence method for the determination of insulin, HPLC method for the determination of HbA1c, HOMA-IR by homeostasis model as- sessment. Results HbA1c, FPG, 2hPG, HOMA-IR, FFA and 2hFFA values of each index in T2DM group were significantly higher than normal control group, the differences were statistically significant (P 〈 0.05). In the T2DM group, HbA1c, FPG, 2hPG, HOMA-IR, FFA and 2hFFA indicators in the poor glycemic control group were significantly higher than the good control group, the differences were statistically significant (P 〈 0.05), FFA, 2hFFA were significantly positive related to HbA1c, FPG, 2hPG, HOMA-IR (P 〈 0.05). Conclusion T2DM in the patients with elevated FFA levels and insulin resistance have a close relationship, and plays an important role in the pathogenesis of T2DM.
