主动免疫恢复T辅助细胞平衡治疗习惯性流产

目的:寻求安全、有效地治疗不明原因习惯性流产(UHA)的方法,并探讨主动免疫治疗前后的细胞因子的免疫功能变化及T辅助细胞的平衡状态。方法:①对35例UHA患者采用丈夫肘静脉血20 m l分离淋巴细胞行主动免疫治疗(妊娠前治疗两次,妊娠后治疗两次);②采用酶联免疫吸附方法对68例UHA患者及30例正常妇女外周血IL-2、IL-4、IL-10、IFN进行检测;③对29例主动免疫后妊娠成功者治疗前后的IL-2、IL-10、Th1/Th2进行检测。结果:①主动免疫组33例妊娠,妊娠成功并分娩29例,妊娠成功率87.88%(29/33),未经主动免疫治疗的对照组妊娠成功率26.67%(8/30),两组相比差异有显著性(P<0.010);②68例UHA患者较正常妇女外周血中IL-2明显升高(P<0.01),IL-10降低(P<0.05),Th1/Th2比值增高(P<0.05);③29例UHA患者主动免疫治疗后妊娠成功者IL-10显著上升(P<0.1),Th1/Th2比值下降(P<0.05)。结论:UHA患者Th1、Th2平衡异常,主动免疫治疗可诱导Th1向Th2转移,恢复Th1、Th2之间的平衡,可用于UHA的治疗。

Initiative immunotherapy of balance T assistant cell for primary habitual abortion

Objective:To search a safe and effective method to cure the unexplained habitual abortion(UHA) of ambiguous reasons,and investigate the change of the cell's immunity function and the balance situation of T assistant cell during before and after initiative immunotherapy.Methods:①Firstly,for 35 UHA sufferers,collect husband's leukocytes suspension in treatment of habitual abortion(normally,for two times before their wives are pregnant,and two times after the pregnancy);②Adopting the immune adsorption measurement with enzyme,for 68 UHA sufferers and 30 healthy women,and examining the periphery blood by IL-2,IL-4,IL-10,IFN-γ..③Exam IL-2,IL-10 and TH1/TH2 of 29 successful pregnant women who accepted the initiative immunotherapy before and after treatment.Results:①Among 33 pregnant women who accepted the initiative immunotherapy,29 pregnant women got childbearing successfully,and the success rate was 87.88%(29/33).And the success ratio of comparing group without the initiative immunotherapy was 26.67%(8/30),and the different results between these two were distinct(P<0.01);②Comparing the 68 UHA sufferers with the healthy women,the IL-2 in periphery blood increased distinctly(P<0.01),and the IL-10 reduced(P<0.05),the ratio between TH_1 and TH_2 increased(P<0.05);③For 29 successful childbearing women who accepted the initiative immunotherapy,their IL-10 increased distinctly(P<0.01),the radio between TH_1 and TH_2 reduced(P<0.05).Conclusion:Because the balances of TH1 and TH2 between UHA sufferers and THA sufferers are deviant,using the initiative immunotherapy can induce the TH1 transfering to the TH2,and then the balance between TH1 and TH2 would come back,which can be used for the cure of UHA.
