经阴道三维超声在输卵管间质部妊娠诊疗中的应用价值

摘    要:
目的:探讨经阴道三维超声在输卵管间质部妊娠诊疗中的应用价值。方法:对二维超声疑为输卵管间质部妊娠的患者共56例,行阴道三维超声检查,根据三维超声所见,将输卵管间质部妊娠分为近宫腔型(Ⅰ型)和非近宫腔型(Ⅱ型),其中Ⅰ型11例,Ⅱ型45例。结果:所有患者行宫腹腔镜联合检查,11例Ⅰ型输卵管间质部妊娠中2例误诊,45例Ⅱ型确诊。9例确诊Ⅰ型输卵管间质部妊娠患者行宫腹腔镜联合监视下输卵管间质部妊娠吸引术,5例获得成功,其余4例Ⅰ型和所有Ⅱ型输卵管间质部妊娠行腹腔镜下输卵管及一侧宫角切除术。结论:经阴道三维超声可以准确获得输卵管间质部妊娠病灶与宫腔关系,为临床选择不同的手术方法提供依据。

Abstract:
Objective:To evaluate the value of 3D transvaginal ultrasound in interstitial tubal pregnancy. Methods: Performance of 56 interstitial tubal pregnancy by 2D and 3D transvaginal ultrasound, And divided into Type I (Type of near the uterine cavity) and type (type of non-near the uterine cavity), including 11 type I and 45 type Ⅱ. Results:All patients were performed by laparoscopy combined with hysteroscopy, and 2 cases of type I was misdiagnosis. The 5 cases of Type I were succeed to performance transcervical suction in laparoscopic and hysteroscopic guidance. Conclusion 3D transvaginal ultrasound can get the relationship of the lesion and uterin cavity, and provide the guidance for clinical treatment.
