~(99m)Tc-HL9l乏氧显像对原发性肺癌的诊断评价

摘    要:为了探讨99mTcHL9l乏氧显像对原发性肺癌的诊断效能。对25例拟诊断原发性肺癌患者,术前进行放射性核素99mTcHL9l肿瘤乏氧细胞显像,以病理结果为金标准,分析99mTcHL9l显像对原发性肺癌的诊断价值。术后病理结果显示,原发性肺癌19例,转移性肺癌1例,良性病变5例(肺结核球3例,炎性假瘤和错构瘤各1例)。19例原发性肺癌中有17例显像阳性,5例良性病灶中2例显像阳性。1例转移性肺癌呈阳性显像。99mTcHL9l显像对原发性肺癌诊断的灵敏度为89.5%(17/19),特异性为60%(3/5),准确性83.3%(20/24)。初步研究结果提示,99mTcHL9l显像对原发性肺癌的诊断有较好的应用价值,99mTcHL9l显像能直观地显示肺癌的乏氧情况。

Research on the diagnostic value of ~(99m)Tc-HL9l hypoxie imaging in primany lung cancer
Abstract:To investigate the clinical value of 99mTc-HL9l hypoxic imaging in the diagnosis of primary lung cancer. Twenty-five patients with primary lung cancer underwent 99mTc-HL9l hypoxic imaging before surgery. The diagnosis of primary cancer was confirmed by pathological diagnosis study after surgery. The diagnostic effectiveness of 99mTc-HL9l in primary lung cancer was analyzed. The pathological diagnosis showed that 19 patients with primary lung cancer, 1 patient with metastatic lung cancer and 5 patients with benign tumor (3 cases with tuberculomas, 1 case with inflammatory,1 case with hamartoma). Of 19 lung cancer patients, 17 cases were true positive. In 5 cases with benign foci, 2 of them were found imaging positively (2 cases fake positive , 3 cases true negative). One patient with metastatic lung cancer was found imaging positively. The sensitivity, specificity and accuracy of 99mTc-HL91 imaging in primary lung cancer were 89.5%, 60.0% and 83.3%, respectively. Conclusion 99mTc-HL9l imaging is a useful method in the diagnosis of primary lung cancer. The hypoxic status of lung cancer can be shown by 99mTc-HL9l imaging.
