火山口征及微钙化对乳腺浸润性癌和原位癌的诊断价值

摘    要:目的探讨自动乳腺全容积成像(ABVS)冠状面火山口征以及微钙化对乳腺浸润性癌(IDC)和乳腺原位癌(DCIS)的鉴别诊断价值。方法回顾性分析102个经穿刺或手术病理证实的乳腺癌病灶。按照乳腺癌的浸润与否分为乳腺浸润性癌(IDC)和乳腺原位癌(DCIS)。通过观察ABVS冠状面图像,与病理结果进行对照分析,分析ABVS火山口征以及微钙化对两种乳腺癌的鉴别诊断价值。结果 101例乳腺癌病灶中,IDC在ABVS冠状面出现火山口征的阳性率(51.2%)高于DCIS (10%),P<0.05。IDC在肿块内(81.9%)检出微钙化的比例高于DCIS (54.3%)、在导管内检出微钙化的比例(18.1%)低于DCIS (45.7%),P<0.05。IDC出现散在少量微钙化的比例为(73.2%)高于DCIS (40%),P<0.05。DCIS出现钙化簇的阳性率(35%)高于IDC(14.6%),P<0.05。结论 ABVS冠状面火山口征以及微钙化的检出部位、数量和钙化簇的出现,对于IDC、DCIS有鉴别诊断的价值。

Abstract:Objective To investigate the Diagnostic value of the crater sign and microcalcification by automated breast volume scanner(ABVS)in diagnosis of invasive ductal carcinoma and ductal carcinoma in situ.Methods A retrospective analysis of 102 cases of breast cancer lesions confirmed by puncture biopsy or pathology.According to the infiltration of breast cancer,it is divided into breast invasive carcinoma(IDC)and breast in situ carcinoma(DCIS).By observing the ABVS coronal image and comparing the pathological results,analyze the differential diagnosis value of the crater sign and microcalcification by ABVS.Results Among 101 breast cancer lesions,the positive rate of crater sign on coronary section by ABVS in IDC was higher(51.2%)than that of DCIS(10%),P<0.05.The percentage of microcalcification detected in the mass of IDC(81.9%)was higher than that of DCIS(54.3%),and the percentage of microcalcification detected in the catheter(18.1%)was lower than that of DCIS(45.7%),P<0.05.The proportion of IDC with a small amount of microcalcification was(73.2%)higher than DCIS(40%),P<0.05.The positive rate of calcification cluster in DCIS(35%)was higher than that in IDC(14.6%),P<0.05.Conclusions The crater sign on coronary section by ABVS and the detected location and the detected quantity of microcalcification and the appearance of calcification clusters are valuable for the differential diagnosis of IDC and DCIS.
