超声引导微创旋切术与开放性手术在乳腺肿瘤治疗中的应用价值

目的对比分析超声引导微创旋切术与开放性手术在乳腺肿块治疗中的应用效果与安全性。方法将92例乳腺肿块患者随机分为两组(n=46),A组在超声引导下行微创旋切术,B组行开放性切除术。对比两组的手术时间、术中出血量、住院时间、瘢痕长度、完全切除率以及术后并发症发生率、满意度等。结果A组患者的手术时间、切口愈合时间、瘢痕长度、术后住院时间均显著短于B组,术中出血量显著低于B组(P<0.05)。A组直径<2.5 cm、≥2.5 cm病灶的完全切除率均为100.00%,均显著高于B组的80.00%和71.43%(P<0.05)。与B组相比,A组患者术后的局部血肿、乳房变形、局部瘀斑发生率更低,术后12、24 h的VAS评分明显更低,治疗满意度、乳房外观满意度评分更高(P<0.05)。结论相比开放性手术,超声引导微创旋切术治疗乳腺肿瘤的疗效更好、创伤更小、并发症更少、美观性更好。

Applied Value Research of Ultrasound-guided Minimally Invasive Circumcision and Open Surgery in the Treatment of Breast Masses

Objective To compare and analyze the effect and safety of ultrasound-guided minimally invasive circumcision and open surgery in the treatment of breast masses.Methods 92 patients with breast mass were randomly divided into two groups(n=46).Group A received minimally invasive circumcision under the guidance of ultrasound,and group B received open resection.The operation time,intraoperative hemorrhage,hospitalization time,scar length,complete resection rate,postoperative complications and satisfaction were compared between the two groups.Results The operation time,incision healing time,scar length and postoperative hospitalization time of group A were significantly shorter than those of group B,and the intraoperative bleeding volume was significantly lower(P<0.05).The complete resection rate of lesions with diameter less than 2.5 cm and more than 2.5 cm in group A both were 100.00%,which were significantly higher than 80.00%and 71.43%in group B(P<0.05).Compared with group B,the incidence of local hematoma,breast deformation and local ecchymosis in group A was lower,VAS score was significantly lower at 12 h and 24 h after operation,treatment satisfaction and breast appearance satisfaction score were higher(P<0.05).Conclusions Compared with open surgery,minimally invasive circumcision guided by ultrasound has better curative effect,less trauma,less complications and better aesthetics.
