非特异度乳腺炎的声像图表现及鉴别诊断

目的探讨非特异度乳腺炎的声像图表现、病理特征及临床特征。方法回顾性分析我院109例经病理确诊的非特异度乳腺炎,其中肉芽肿性小叶乳腺炎(GLM)78例,浆细胞乳腺炎(PCM)8例,糖尿病性乳腺病(DMP)23例。比较三种不同病理类型乳腺炎的声像图表现、病理特征以及临床特征。结果 84.6%(66/78)GLM初始病灶位于外象限,74.4%(58/78)病灶周围组织回声增强,11.5%(9/78)乳头内陷或畸形。75.0%(6/8)PCM初始病灶位于乳晕区,25.0%(2/8)病灶周围组织回声增强,37.5%(3/8)乳头内陷或畸形。82.6%(19/23)DMP初始病灶位于外周,17.4%(4/23)周围组织回声增强,13.0%(3/23)病灶区仅见声衰减,52.2%(12/23)DMP患者有糖尿病史。结论非特异度乳腺炎超声图像表现复杂多样且缺乏特异度,但三者在发病年龄、好发部位、周边回声、乳头输乳管是否发育异常及临床表现方面存在差异。

Ultrasonographic Features and Differential Diagnosis of Nonspecific Mastitis

Objective To analyze the ultrasound Imaging Features,pathology characteristic,clinical characteristic of non-specificity mastitis.Methods Ultrasound findings of 109 patients who were diagnosed as non-specificity mastitis,including 78 Granulomatous lobular mastitis(GLM),8 Plasma cell mastitis(PCM)and 23 Diabetic mastopathy(DMP)by pathology,were analyzed retrospectively.Results 84.6%(66/78)of GLM initial lesions were located in the periphery of the breast,74.4%(58/78)of the surrounding tissues were enhanced,and 11.5%(9/78)of the nipples were inverted or deformed.75.0%(6/8)PCM initial lesions were located in areola,25.0%(2/8)surrounding tissue echoes were enhanced,37.5%(3/8)nipple inversion or malformation.82.6%(19/23)DMP initial lesions were located in the periphery of the breast,17.4%(4/23)of the surrounding tissues were enhanced,13.0%(3/23)posterior echo attenuation,52.2%(12/23)with a history of diabetes.Conclusions Ultrasound images of non-specific mastitis are complex and lack of specificity.However,there are differences in age,predisposing sites,peripheral echoes,papillary duct dysplasia and clinical manifestations.
