三种革兰氏阴性菌的耐药性变迁与抗菌药物用量变化的相关性

目的探讨呼吸病区抗菌药物使用与病原菌耐药水平的量化关系,为合理使用抗菌药物,降低呼吸病区细菌耐药水平提供科学依据。方法采用回顾性调查和前瞻性研究相结合的动态监测方法,阐明呼吸病区抗菌药物DDDs与病原菌耐药水平的量化关系。结果 2004～2009年11种抗菌药物的使用频度变化不一,青霉素、头孢他啶、阿米卡星、环丙沙星的使用频度总体是下降的;从产ESBL克雷伯氏菌属和铜绿假单胞菌的耐药性数据可以看到,对环丙沙星、左氧氟沙星、庆大霉素、阿米卡星、头孢他啶的耐药率这六年总体来说是随着时间而下降。产ESBL克雷伯氏菌属和铜绿假单胞菌分别对于阿米卡星、左氧氟沙星、头孢他啶的耐药率与抗菌药物的使用频度之间存在显著正相关。结论抗菌药物的不合理使用和滥用其实是造成细菌耐药性的主要原因。数据显示抗菌药物的使用量与细菌耐药性有相关性关系,同时革兰氏阴性菌ESBL（＋）的发生率是与第三代头孢类的使用量增加有关。
