78例子宫内膜癌相关多原发癌临床分析

目的 探讨子宫内膜癌相关多原发癌的临床病理特点。方法 我院1990年～2004年共收治子宫内膜癌患者1046例，回顾分析其中78例多原发癌的临床病理特点。结果 78例多原发癌占同期子宫内膜癌的7．5％，其中子宫内膜癌合并卵巢癌、宫颈癌、大肠癌、乳腺癌和其他少见部位恶性肿瘤分别为26例（33．3％）、16例（20．5％）、16例（20．5％）、12例（15．4％）、8例（10．3％）。发生第一原发癌和第二原发癌中位间隔时间为7年3个月。多原发癌中子宫内膜癌中位发病年龄51．6岁，临床病理特点和单发子宫内膜癌相似。除大肠癌外，合并其他部位肿瘤显著降低子宫内膜癌的5年生存率（总5年生存率26．9％）。结论 子宫内膜癌相关多原发癌的预后差，应予重视。

Clinicopathological analysis of 78 cases of multiple primary neoplasms associated with endometrial carcinoma

Objective To analyze clinicopathological features of multiple primary neoplasms associated with endometrial carcinoma.Methods A total of 78 patients suffered from multiple primary neoplasms among 1046 cases of endometrial carcinomas were admitted in our hospital from 1990 to 2004.Their clinopathological data were retrospectively studied.Results The incidence of multiple primary neoplasms was 7.5% among those with endometrial carcinoma.Besides endometrial carcinomas the other neoplasms were ovarian carcinomas (26/78, 33.3%), cervical carcinomas (16/78, 20.5%), rectosigmoid carcinomas (16/78, 20.5%), breast carcinomas(12/78, 15.4%)and other sites neoplasms (8/78, 10.3%).The median interval between the first and the second primary neoplasms was 7.25 years.The median age of endometrial carcinomas patients was 51.6 years old, and the clinical and pathologic features of the endometrial carcinomas of multiple primary neoplasms were similar to primary endometrial carcinomas alone.Except rectosigmoid carcinomas, if endometrial carcinomas patients had suffered or suffered from other neoplasms, the 5-year survive rate of endometrial carcinomas (26.9%) reduced significantly.Conclusion The patients of multiple primary neoplasms associated with endometrial carcinoma present poor prognosis and need more recognition.
