摘    要:目的探讨血清微小RNA(miRNA)-10b和miRNA-34a在子宫内膜异位症(EMT)患者中的表达及与术后复发关系。 方法选择就诊于承德医学院附属医院妇科2017年1月~2019年12月子宫内膜异位症患者126例作为观察组;另选择同期健康育龄期女性89例作为对照组。所有EMT患者均随访1年,采用门诊、电话或微信等方式,观察患者术后复发情况。采用实时荧光聚合酶链反应(RT-PCR)法测定血清miRNA-10b和miRNA-34a表达。比较EMT组与对照组血清miRNA-10b和miRNA-34a相对表达量;复发组与未复发组血清miRNA-10b和miRNA-34a相对表达量;采用ROC曲线分析miRNA-10b和miRNA-34a对EMT术后复发预测价值;采用多因素Logistic回归分析EMT术后复发危险因素;采用Pearson分析miRNA-10b和miRNA-34a与术后复发相关性。 结果EMT组血清miRNA-10b和miRNA-34a相对表达量高于对照组(P<0.05)。随访1年,均完成随访,复发29例,未复发97例。复发组血清miRNA-10b和miRNA-34a相对表达量高于未复发组(P<0.05)。经ROC曲线分析显示,miRNA-10b预测EMT术后复发中,敏感度为68.75%,特异度为69.23%;miRNA-34a预测EMT术后复发中,敏感度为66.67%,特异度为57.14%。经多因素Logistic回归分析显示,miRNA-10b和miRNA-34a为影响EMT术后复发危险因素。经Pearson分析,miRNA-10b和miRNA-34a与术后复发呈正相关。 结论血清miRNA-10b和miRNA-34a在EMT患者中表达升高,与术后复发呈正相关,且为影响EMT术后复发危险因素,及对EMT术后复发预测敏感度和特异度良好。

Abstract:ObjectiveTo investigate the expression of serum microRNA (miRNA)-10b and miRNA-34a in patients with endometriosis (EMT) and to analyze their relationship with postoperative recurrence. MethodsOne hundred and twenty-six patients with endometriosis treated at our hospital from January 2017 to December 2019 were selected as an observation group, and 89 healthy women of childbearing age who visited our hospital from January 2017 to December 2019 were selected as a control group. All EMT patients were followed for 1 year, using outpatient, telephone, or WeChat method to observe postoperative recurrence. Real-time fluorescent polymerase chain reaction (RT-PCR) was used to determine the expression of miRNA-10b and miRNA-34a in serum. The relative expression of miRNA-10b and miRNA-34a in serum was compared between the observation and control groups, and between the relapsed and non-relapsed groups. The receiver operating characteristic (ROC) curve was plotted to analyze the predictive value of miRNA-10b and miRNA-34a in postoperative recurrence of EMT. Multivariate logistic regression was used to identify the risk factors for postoperative recurrence of EMT. Pearson's correlation analysis was perform to explore the correlation between miRNA-10b and miRNA-34a and postoperative recurrence. ResultsThe relative expression of miRNA-10b and miRNA-34a in the EMT group was significantly higher than that in the control group (P<0.05). At the 1-year follow-up, 29 cases relapsed and 97 did not. The relative expression of miRNA-10b and miRNA-34a in the relapsed group was significantly higher than that of the non-relapsed group (P<0.05). ROC curve analysis showed that miRNA-10b had a sensitivity of 68.75% and specificity of 69.23% in predicting recurrence of EMT; miRNA-34a had a sensitivity of 66.67% and specificity of 57.14% in predicting recurrence of EMT. Multivariate logistic regression analysis showed that miRNA-10b and miRNA-34a were risk factors for recurrence of EMT. According to Pearson's correlation analysis, miRNA-10b and miRNA-34a were positively correlated with postoperative recurrence of EMT. ConclusionSerum miRNA-10b and miRNA-34a are elevated in EMT patients and are positively correlated with postoperative recurrence of EMT; they are risk factors for recurrence of EMT and have good sensitivity and specificity in predicting recurrence after EMT.
